照顧經理 (SW III) (合約)〈編號: EOL/90824〉

職　責 Responsibility：

• 提供綜合社區安寧照顧服務予晚期患者及照顧者 (負責擬定個案照顧計劃及跟進)
• 與賽馬會安寧頌計劃及社區持份者聯繫
• 責統籌安寧義務工作
• 五天工作，按工作需要星期六、日或需當值
• 工作地點 : 香港島

申　請　資　格 Qualification：

• 註冊社工，持有社工文憑或以上學歷
• 香港中學文憑試五科合格 (包括中英文) 或同等學歷
• 有晚期寧養服務工作經驗者為佳
• 富責任感、主動積極、良好溝通能力

待　遇 Treatment：

• 五天工作，按工作需要星期六、日或需當值
